(hereinafter collectively and indistinctly referred to as "FM", "we", "our" or "us") is an international reference centre in Gynaecology, Obstetrics and Assisted Reproduction, belonging to the International Assisted Reproduction Group owned by Futurelife, A.S., with registered office at Na příkopě 1047/26, Nové Město, 110, Prague, Czech Republic ID No.: 026 25 067 (hereinafter "Futurelife").
We provide personalised care with professionals working as a team, based on a strong human bond between doctor and patient. All specialists coordinate in order to offer couples a service that combines the latest technology with the most advanced diagnostic and treatment methods.
We attach great importance to the privacy of individuals, whether visitors to our website https://fertilitymadrid.com/ (hereinafter the "website") or any other affected parties who provide us with information through it, whose data are processed by FM as part of the provision of services (hereinafter and collectively "you" or the "user", and in plural form "you" or the "users").
In this regard, FM undertakes to process your data in accordance with the personal data protection regulations applicable and in force at all times. Specifically, FM will comply with the provisions laid down in current Spanish legislation, as well as those of REGULATION (EU) 2016/679 OF THE EUROPEAN PARLIAMENT AND OF THE COUNCIL of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, repealing Directive 95/46/EC, and any other relevant regulations in force.

1. Who is responsible for the processing?
The data controller is the Futurelife´s operational entity - FL Service, s.r.o., ID no. 04213190, with registered office at Na Poříčí 1047/26, Nové Město, 110 00 Prague 1, Czech Republic, registered in the Commercial Register maintained by the Municipal Court in Prague, file number C 243578.
2. What are the users’ data protection rights?
Users may exercise the following rights:

Should you exercise this right in relation to data necessary for FM to provide services through the website, FM will be obliged to terminate its relationship with you, proceeding to deregister you without any entitlement to claim.
To exercise your rights, users must send written communication to FM’s registered office or to the email address indicated in section 1 of this Privacy Policy, in both cases including a photocopy of your ID card or an equivalent identity document.
3. How can you file a complaint with the Spanish Data Protection Agency?

Complaint before the Supervisory Authority: If you believe FM is improperly handling your personal data, you may lodge a complaint with FM’s DPO or with the relevant Supervisory Authority. In Spain, this is the Spanish Data Protection Agency (AEPD).
4. In what cases do we collect personal data about you and what type of data?
For the purposes of this Privacy Policy, “personal data” means any information that identifies you or may be used to identify you, such as your name, address, telephone number or email address.
If you decide to provide us with your personal data, FM and any Futurelife entity may have access to the data included in each form you complete or those you provide through the different email accounts FM makes available for contacting us.

FM is present on some of the main social networks, where it acts as controller regarding data published by FM.
FM will process data on each social network according to the rules established by that platform. Therefore, unless otherwise indicated by FM, we may inform followers on the relevant social network about our activities, events and other related topics, including customer service, through the channels provided by each network.
FM will not extract personal data from social networks unless you expressly give us your consent.
13. Special categories of personal data
We will only collect and process your personal data relating to health, race or ethnic origin, genetic data, biometric data for the exclusive purpose of identifying a natural person, sex life or sexual orientation if you have explicitly provided them to us.
Other categories of special personal data, such as political opinions, religious or philosophical beliefs or trade union membership, will neither be collected nor processed.
14. Child use policy
This website is not designed for or directed at children under eighteen (18) years of age without parental or legal guardian authorisation. We do not knowingly collect information from such individuals.
