In vitro fertilization (IVF) is one of the most widely used techniques in assisted reproduction to help achieve pregnancy. At Fertility Madrid, we carry out this procedure by combining cutting-edge technology with personalized care, from ovarian stimulation to embryo transfer. It is a recommended option in cases of complex infertility, offering high success rates and solutions tailored to each patient.

IVF Treatment Process

Before starting IVF, we determine the best technique for each patient—whether using your own eggs and your partner’s sperm, or using donors. At Fertility Madrid, we tailor the treatment to your individual needs.

Once the plan is agreed, the treatment follows these steps:

Choosing the right IVF technique

We begin by selecting the method that best suits your needs. Currently, four options are available:

  • IVF with your own eggs and your partner’s sperm
  • IVF with your own eggs and donor sperm
  • IVF with donor eggs and your partner’s sperm
  • Double donation (both eggs and sperm from donors)

Ovarian stimulation

This uses hormonal medication to stimulate multiple egg follicles. Normally, only one egg matures naturally—but with medication, several follicles develop, providing more mature eggs.

Follicle retrieval

This is a simple procedure done under sedation in the operating theatre. The antral follicles are aspirated to retrieve the eggs.

Fertilisation of the eggs

In the IVF lab, embryologists locate the eggs in the follicular fluid. On the same day as retrieval (Day 0), the sperm sample (partner or donor) is prepared, and fertilisation occurs.

This can be done in two ways:

  • Conventional IVF, where egg and sperm unite naturally, or

  • ICSI (Intracytoplasmic Sperm Injection), where a single sperm is injected into each egg.

Embryo culture

After fertilisation, embryos are cultured in a Time-Lapse GERI incubator. This state-of-the-art incubator takes photos every 5 minutes, creating a time-lapse video—so we don't have to remove embryos for observation. Embryos develop in individual compartments for each patient and stay in culture for 3–6 days, depending on the case.

Embryo transfer

Once high-quality embryos are selected, either they are transferred to the uterus or vitrified. Transfers take place in the operating theatre—close to the lab—to protect embryo development. Typically, one or two embryos are transferred.

Spanish law allows up to three embryos, but we transfer no more than two, and only if the patient gives informed consent acknowledging the risks of twins. Otherwise, we strongly prefer transferring a single embryo to reduce multiple birth risks.

If there are remaining embryos, they are vitrified—freezing "biological time". These can be used in future cycles without ovarian stimulation; only endometrial preparation is needed.

Vitrification of remaining embryos

Sometimes, all of the embryos created, or the ‘remaining’ ones, are vitrified. In this way, we are able to stop the biological clock. These embryos can then be used in later cycles without the need to go through ovarian stimulation again.

In such cases, the only step required is to prepare the patient’s endometrium.

  • Today marks one year since we walked through the doors of the clinic to begin the egg donation process. And what a year it’s been... So many things have happened (many of them quite strange) that it feels both short and long at the same time.

    I had already been there in February for an initial consultation that my husband couldn’t attend, and I left with a really good feeling. I liked the facilities, but above all, I loved how the gynecologist treated me. Even so, since I’m the most indecisive person in the world, it took me over a month to make up my mind. The fact that he hadn’t been able to come didn’t help, because even though he told me from the start that we’d go wherever I wanted, his impressions meant a lot to me.

    As you know, during that month I went over every single pro and con of all the clinics we had visited. I weighed the facilities, the staff, the prices, the guarantees, the location… This wasn’t the cheapest clinic or the closest, but something inside me told me it was the right one. So I told my husband, he agreed, and we made an appointment to finally get started.

    The pandemic was already making itself felt in Spain. Schools had closed in Madrid, and the situation was, at the very least, worrying. We actually had the appointment set earlier and postponed it by a couple of days out of fear… But in the end, we decided to go. We didn’t know what the future held, how long this would all last, whether things would return to normal soon or more places (including fertility clinics) would close and everything would be delayed. With so much uncertainty, we felt we had to at least take the first step. I think we made the right call — just two days later, the state of emergency was declared and everything shut down.

    One year later, with the pandemic still affecting our lives far more than we would have imagined back then, I’m pregnant — and infinitely grateful.
    Grateful to the entire team at Fertility Madrid for making this possible — to our gynecologist, the nurses, the embryologists, the reception staff, the psychologist… And of course, grateful to that anonymous donor, because without her, our little one wouldn’t be on the way.
